They are medication experts, specializing in medication use in older adults. As such, pharma- cists servicing personal care homes are relied upon to make recommendations regarding dose adjustments and therapeutic substitutions. They also provide opinions, information and recom- mendations on a variety of other resident-related medication issues. Since residents of personal care homes have multiple chronic and acute conditions requiring numerous medications, pharmacists must promptly respond to any changes that affect the resi- dent's status in order to provide the highest quality pharmacy services. Manitoba pharmacists servicing personal care homes believe in a resident-centered approach. They attend medication reviews with the interdisciplinary team of physicians and nurses for all personal care home residents. This ensures medication needs are reviewed on a regular and consistent basis, promoting optimum health outcomes. They also provide a detailed explanation of medications to the residents themselves, or their caregivers, as needed when concerns arise. Pharmacists help contribute to optimal resident care by providing education to nurses and other allied health-care professionals in the form of live presentations and informational bul- letins on a variety of medication and disease related topics.
